Thank you for your interest in participating in a Maya Practitioner Research Survey.

We are very protective of our panelists’ data and take privacy very seriously. We have created this Privacy and Data Use Policy (“Policy”) to help answer questions you may have about how we protect and use your information. 

Please note that this privacy policy applies only to the Maya PBC Market Practitioner Research exercises, found through the Site and no other services provided by Maya PBC.

Please also note that other services provided by Maya PBC are governed by their own policies.

This notice replaces any other statement, whether written or oral, made to you about our practices with respect to the collection and use of Personally Identifiable Information through our Maya Practitioner Research Surveys.

A. We protect the identities of our participants. 

We take all commercially reasonable efforts to protect the privacy of our research participants and keep the information they supply secure as detailed in this policy. Please note that no system is 100% secure, and we cannot provide an absolute guarantee regarding data security. As part of this effort, we always allow respondents to choose to answer anonymously in our research activities.

As used in this Policy, “Personally Identifiable Information” refers to information that by itself – like your first and last name, home postal address or personal email address – or in combination with other information, could identify a specific individual.

If we contact you – by phone, email, text, or postal mail – we will do so for one of the following reasons: to validate answers you gave; to update our records to ensure your information is correct; to deliver incentives related to participating in research, or to offer you expanded research opportunities.

B. What information do we collect? 

Maya PBC researches the psychedelic therapy field and its stakeholders. In order to create our research insights, we collect information in the following ways:

i. Information you give us
When you become a research participant, you provide us with information such as your name and email address. Through participation in our panels and surveys, we collect information about you, your organization/business, and your attitudes and opinions.

ii. Publicly available information
In addition to panel and survey responses, we may elect to conduct our own web-based research from publicly available sources such as, but not limited, to professional website(s) and web presence. 

iii. Cookies
We collect cookies while people are on www.mayahealth.com. We may use cookies and other technologies to help recognize your browser or device, maintain your preferences, provide certain Site features, and collect Information about interactions with our Sites, our content, and our communications. We do not link any of the information stored in cookies to any Personally Identifiable Information coming from the survey. 

Our survey platform, Checkbox, also collects cookies. Checkbox does not link the information they store in cookies to any personally identifiable information you submit while on their site. Please see Checkbox’ privacy policy for more details on how they store cookies.

C. How will we use the information you and other panelists provide us? 

i. Personally Identifiable Information (PII)
We will never use your Personally Identifiable Information to advertise, promote, or market goods or services directly to you without your explicit consent and we do not allow our clients to market directly to you.

We will never sell your Personally Identifiable Information to any third parties.

ii. Non-PII / Aggregated Data
We use the information we collect to create a variety of research insight reports for our clients. We will also release high-level findings on our website, and share these findings with various  press and media. At our discretion, we may sell or share this aggregated data with third parties.  

In the reports that we create for our clients, we use tools and methods to minimize the possibility of identifying an individual participant's data points in the reports that we create for our clients. This means that data points will not be explicitly labelled as being from a specific participant unless that participant requested that their data be identified as part of their participation in the research. 

In all our research, there is the opportunity to provide us with permission to call-out your data within our reports should it be relevant. For further clarification, opting into this call-out option would be giving Maya PBC permission to disclose certain Personally Identifiable Information within the report.

We use de-identified data response data about former panelists for market research purposes (e.g., tracking trends and changing practitioner protocol over a period of years), or to intentionally clutter data sets containing current panelists in order to help protect their identities. For example, we combine the responses we collect from you with the responses of others to produce aggregated reports. 

Please remember that storage and communication of personal data may not always be one-hundred percent secure.

D. How long do we keep your personally identifiable information? 

Due to the nature of the research we are conducting we permanently remove any connection between personally identifiable information and survey responses within 60 days of closing the response collection period. All response data is also permanently deleted from our survey platform provider (www.checkbox.com) at this time.

Broader Non-PII survey response data is stored for as long as needed for our research and analyses purposes and in accordance with law. 

E. Data retention and security 

We store your information for as long as needed for our research and analyses purposes and in accordance with law. Your information may be stored in the United States or elsewhere but in any event will be stored using administrative, managerial and technical measures to protect its confidentiality. Please remember that storage and communication of personal data may not always be one-hundred percent secure. The security of the data is dependent on the security of the third parties that store it (please find their policies here Amazon and Checkbox). 

F. Do you provide my Personally Identifiable Information to anyone? 

Maya PBC works with a survey platform provider (www.checkbox.com) to distribute research and collect responses. Checkbox will retain data collected through their software until Maya PBC deletes it, per their privacy policy. Maya PBC will download and delete all data from the Checkbox platform within 60 days of closing our research.

Maya PBC will only provide your  Personally Identifiable Information to a third party if the law requires it or if it allows us to exercise our legal rights or take action regarding potentially illegal activities, or to protect the safety or rights of any person or legal entity. If all or part of our company is sold, merged or otherwise transferred, we may transfer your information as part of that transaction and will take steps to ensure that your information is protected as described in this notice.

G. Participation is always voluntary

You may contact us if at any time you would like to see the Personally Identifiable Information we hold about you. You can also ask us to correct or update this information by contacting us.

We sincerely appreciate your willingness to serve as a research participant.

H. We may change this Privacy and Data Use Policy and our data collection and management practices. 

We reserve the right to update and make changes to this Policy and to our practices in collecting and handling personally identifying information. If we were to make a material change,  we will notify you at the most recent email address we have on file and we will provide you with access at least 30 days prior to its implementation.

You may view the currently effective Privacy Notice at any time by checking the Privacy and Data Use Policy posted on the Maya PBC Research website. Your continued participation in our research after the effective date of any changes will constitute your acceptance of all effective changes.
